To Obtain an Appointment
Your Primary Care Provider must refer you to see one of our Providers. Once you are referred you can request an appointment through our scheduling department, you may use the web site or call:
(225) 769-2295 between 8:00am and 4:00pm Monday through Friday.
However, if you have sudden onset of symptoms that require immediate attention, please call our office, and we will schedule an appointment as soon as possible.
If you have an emergency please call 911 during regular business hours. For after-hours emergencies please see our Emergencies page.
Cancellations
Pediatric Surgery Of Louisiana requests that you call (225) 769-2295 and provide at least 24 hours’ notice when cancelling or rescheduling an appointment.
Appointment Scheduling - Process
To schedule an appointment please call: 225-769-2295 between 8:00am and 4:00pm Monday through Friday.
Please have your insurance card available when making your appointment. Many managed care insurance plans require a primary care physician referral prior to making a specialist appointment to ensure payment of full benefits. If your plan requires a referral, please have your referral number when scheduling your appointment.
If you are unable to keep your appointment, please call as early as possible to cancel or reschedule.
Check-In Process
All patients need to check-in with the receptionist so that we know you have arrived for your appointment.
The staff will verify your insurance information at each visit, so please bring your insurance card and notify the staff of any changes in your coverage. The staff will also collect any co-payments or portions of your bill that are not covered by your insurance for that visit.
For your convenience, please complete the new patient registration forms which you can obtain through the link below and bring them with you at the time of your appointment.
Already Signed Up?
If you are scheduled to see one of our physicians, please bring a copy of any pertinent labs or x-rays (either through films or a computer disc).
Please inform us at the time of your visit if you were referred by. We will promptly send a report of our consultation to your physician.
Procedures/Surgery
During your evaluation with our Physicians and staff, it is determined that a procedure/surgery is needed. Our Physicians and staff will discuss the options and will consult with you for the scheduling of the procedure/surgery. You will be provided with the information needed to make a decision and will be have Educational information available to you.
